When all 40 raiders, well now 25 raiders, run back to the instance because someone fucked up. The main source of these came from the player named Prurient.
One can rest assured that if there is a way to wipe in an encounter, CSM has discovered and performed it. A list of classic/common CSM wipes to encounters follows.
Note: Wipe is not officially declared until at least 3 raid members yell "WIOTE!!!"

General Phenomena

- Healers running out of mana, presumably because they did not bring at least fifty Night Dragon's Breaths per Shadowalker's orders.
- Following a wipe, healers getting too close to the boss to resurrect dead players.
- Whitemage has caused this kind of wipe in every tier of raid content to date.
- As Whitemage, turning on TriviaBot at any time, in any channel.

- Having a tank be Mind Controlled into a nearby pack of imps.

- As Fallen, pulling Sulfuron -- while dead. (Don't ask me how he did it.)

- As Blackmage, Frost Nova-ing half the mobs that Jody is kiting around the room.
- As the Orb controller, attempting to break the eggs while they are green, so as to slow down dragonkin spawns.

- As Drexel, hitting Firemaw with Mocking Blow at around 25%.

- As Fallen, lacking the ability to properly Tranq shot the boss.
- As random healers, throwing huge heals on the MT before he gets enough aggro on Flamegor.
Ebonroc
- As Manbeast hitting Ebonroc with Mocking Blow at around 25%.
Nefarian
- As Stormcrow, talking to Lord Victor Nefarius with Turn-In enabled, particularly when the raid is most assuredly not-ready.
- As Vrak, healing so much that you're pulling aggro on newly-spawned mobs. Hrangar would never make such a mistake.
- As Hrangar, spamming Prayer of Healing as Nefarian lands, such that repeated Earth Shocks and every warrior threat-producing ability in the game will not pull him off of you.
Zul'Gurub

- As a full raid of CSM mains, pulling the boss.
Jin'do the Hexxer
- As Padmaja, engaging your warrior alt in a duel to the death while dual-boxing both and one is mind controlled by Jin'do the Hexxer. Not really a wipe, but awesome enough to merit mention.

- As Infinite, speaking to the NPC with the Turn-In mod on. Not as bad as when Stormcrow did it, though.
Moam
- As Taru, repeatedly failing to banish your elemental spawn before it is able to slay multiple healers.
Temple of Ahn'Qiraj
Fankriss the Unyielding
- As Hrangar, healing your group and dealing threatless AOE damage via Holy Nova.
- Employing an incredible raid strategy involving a clothie AOE-tanking 40+ mobs.
C'Thun
- Running circles through the raid on phase one because you're Ell or Mwallace and Shadowalker didn't RP the encounter with you.
- Running circles through the raid on phase one because you're Shadowalker's coworker and Shadowalker didn't RP the encounter with you.
- As Atena, running across the room entrance and getting the raid Eye Beamed prematurely. Not really a wipe, because only Caedmon dies.
- As Shadowalker's coworker, attempting to bandage while in the stomach. Not really a wipe, but probably one in spirit.
- As Carine, attempting to Nature's Swiftness / Healing Touch yourself through C'Thun's Dark Glare. Not really a wipe, but prolifically stupid enough to mention here.
Naxxramas
Instructor Razuvious
- Resist on the initial Mind Control.
- As Blizzard, changing the mechanics of Ice Block.
- As Whitemage, the Mind Controller going LD fifteen times, due to playing in a library. He later lost this job.
- As Whitemage or Zelinn, suffering a resisted Mind Soothe. Only applicable while using a Mind Soothe pulling strategy, which did not make it out of the PTR.
- As Hrangar, taunting another Understudy instead of the boss.
Noth the Plaguebringer
- As a DPS class, DPSing following the blink, getting multiple people killed and depriving the raid of the DPS necessary to complete the encounter.
Grand Widow Faerlina
- As Stormcrow, body-pulling nearby spider trash mobs while the raid is recovering from a wipe.
Maexxna
- As Blackmage, failing miserably at controlling spider adds, most likely by Frost Nova-ing them and standing within melee range.
Patchwerk
- As Redcoast, forgetting to take a Stoneshield Potion, at any time, ever.
- As Zelf, at 95%, noting on Ventrilo "Wow, that's a really big Ignite...".
- Letting the tanks die at the last few percents of the fight, kiting Patchwerk around to 1%, and failing to finish him off, despite battle-rezzing freshly-killed mages.
Grobbulus
- As Prurient, pulling with Immolate -- while clearing to Patchwerk.
- As Zelinn, dispelling Mutating Injection the very second it is cast.
- As Blackmage, feeding Ignites five seconds into the fight and pulling the mob off of Jody.
Gluth
- As Lex, failing to successfully kite zombies for any number of reasons, including but not limited to not having +22 Intellect enchanted on your staff.
Loatheb
- After spending upwards of an hour gaining every conceivable buff in the game, including but not limited to Rend, Onyxia, Dire Maul, Zandalar, and Darkmoon Faire, letting Jody die 45 seconds into the fight.
- As Hrangar, being heal-silenced when it is your turn to heal. Additionally, informing everyone on Ventrilo that you are fizzled for twenty seconds before and afterwards.
- As Hrangar, forgetting that his heal macro was set up to target last week's tank in Fumanchu, and not the current tank in Jody.
Gothik the Harvester
- As any priest, having your deathknight shackle break and instantly charge and one-shot you. Necessarily, CSM must wipe to Gothik in this manner at least five times prior to any successful Gothik kill.
- As Blackmage, repeatedly failing to Frost Nova spawning deathknights.
- As Scorpion, targeting yourself and spamming Chain Heal, but not actually healing the melee taking damage.
The Four Horsemen
- As Blackmage, pulling aggro on one of the Horsemen and subsequently Ice Block-ing, sending the mob running across the room.
- As Blackmage, pulling the mobs prematurely while positioning for Thane.
- As Eaglehorn, being in melee range of a dead horseman, particularly while ankhing. Not really a wipe, but seeing him get plowed so dramatically is worth mentioning.
- As any mage, pulling aggro on Mograine in the first thirty seconds of the encounter.
- As Addat, pulling aggro thirty minutes into the encounter.
- As any warlock.
- Following the tank not the actually rotation. "FUCHI WHERE ARE YOU?"
- As Hrangar, Pablohoney, and Revalation, perishing as a group to Meteor.
- Invisible void zones.
Sapphiron
- As Prurient, Codename, or Scorpion, auto-running into the dragon upon porting to Sapphiron's Lair.
- As a mage, generally sucking at decursing your assigned groups.
- As a druid, by virtue of the inability to heal yourself/your group, then running into Sapphiron's cleave to res your fallen druid comrade.
Gruul's Lair
Gruul the Dragonkiller
- Pulling the boss at a time such that the zone's last trash pull will respawn mid-attempt. Will invariably happen during what appears to be an otherwise flawless attempt.
- As Blackmage, going LD at the start of the fight, killing two other people during Shatter and causing the raid to come up a few percent short. Never mind that he was constantly bitching about never getting into 25-man raids.
Tempest Keep
Kael'Thas
- As any/all warlocks, getting you and your entire class killed while tanking Grand Astromancer Capernian.
Battle of Mount Hyjal
Trash
- As Jumanjii, in a fit of rage, punching and breaking his monitor on one of the last waves.
Anetheron
- As Jumanjii, accepting your battle rez in the wake of a Carrion Swarm.
Azgalor
- Deciding that healing Jody on the pull is overrated.
- As Churagar, having your shield break on the pull.
Archimonde
- As Prurient or Korzik, cratering.
- As Addat, hitting Jody with like a 50k Healing Touch crit on the pull.
- As any priest, dying while having the Spirit of Redemption talent.
Black Temple
Trash
- As Manbeast, landing a crushing blow for 327 on Whitemage while he is desperately trying to free you from Mind Control.
Gurtogg Bloodboil
- As Prurient, repeatedly failing to take your Bloodboil.
- As anyone, repeatedly failing to take your Bloodboil. We don't even know who anymore.
- As anyone with (A), failing to press your announce macro at the appropriate time.
- As Prurient, pulling aggro on the boss.
- As Prurient, falling through the world despite being told not to stand in that spot.
- As Prurient, standing on a wall when you receive the Fel Rage debuff, canceling its effects.
- As Prurient, being unable to summon people to the boss room when trash has respawned and being the only warlock there.
- As Prurient, not soulstoning Zelf, despite being told to during the fight multiple times.
Reliquary of Souls
- As a rogue or warrior, interrupting Deaden.
- As a paladin, not putting Blessing of Salvation on shadowpriests going into phase three.
- As Jumanjii, mangling the boss for like five million in an attempt to become the first person to successfully tank Essence of Desire without the ability, Spell Reflect.
- As Jumanjii, not being in the raid to offtank for when the DPS lets Jody get Spirit Shocked.
- As Manbeast, running down into the room as though the encounter were in its bugged phase two state.
- As Curunir, fucking pulling aggro at the start of phase 3. Isn't this supposed to be the easy part?!
Mother Shahraz
- As the main tank, using Last Stand to survive a mid-air Saber Lash and receiving no further heals before you die from fall damage.
Illidari Council
- As Shadowalker, pulling with Cloak of Shadows and promptly getting destroyed by the casters.
- As Voomenstien, clicking "YES" to the "ARE YOU READY?" check and not being ready.
- As Juicyfrut, facing his back to Veras Darkshadow and taking lots of damage.
Illidan Stormrage
- As Churagar,Jumanjii or Heaton, getting leveled by Illidan's blue eye beam. Many times.
- As Davez, setting up a Curse of Doom to tick seconds after Illidan transitions to demon form.
- As Padmaja, being specced Nether Protection.
